Using the PAT Health Record to Support Health Outcomes

The session Using the PAT Health Record to Support Health Outcomes will focus on the effective use of the Parents as Teachers (PAT) Child Health Record to promote positive health outcomes for children. Participants will learn best practices for accurately completing the Child Health Record and how to use this information to inform future discussions and provide ongoing support to families. The session will also enhance attendees’ confidence in collaborating with parents and caregivers, helping them make informed health decisions for their children. By the end of the session, participants will be equipped with practical tools to strengthen health-focused conversations and support within their visits.
